Your First Visit
For complex medical issues, Dr. Maxwell will schedule at least one hour with you. Please be sure to bring a list of your medications, medical records, consultant's letters, lab results, x-ray, CT and MRI results. If you prefer, you may drop these off, or mail them to us before your appointment. Dr. Maxwell will review them prior to your arrival, if you prefer.
Please arrive at least 15-20 minutes before your scheduled appointment with Dr. Maxwell. This will allow time to complete your patient history forms, and just sit and relax!
If using insurance or Medicare, please bring your card. You will also need to bring your driver's license or another form of identification.
Terri, our dedicated Front Office Manager, will greet you when you come in. Terri has been with us since we founded Northgate Family Practice in 1983.
Debra or Elaine, our dedicated and highly skilled Registered Nurses, will measure your height, weight, blood pressure and temperature. Our initial assessment includes a urinalysis. They will ask about your medical history, and the reason for your visit.
Please feel free to bring a list of any questions you may have. Dr. Maxwell is always
glad to answer your questions.
During your first visit with Dr. Maxwell, your medical history will be reviewed and an initial medical exam will be done. Dr. Maxwell will recommend any further testing, and all treatment options will be discussed in detail.
